Licensed Practical Nurse (LPN) Position Summary: The Licensed Practical Nurse (LPN)LPN will function as amembertheIntegrated Health CareClinical Team inassistingwith a full range of nursing/medical services forclients. Under the direction of the registered professional nurse the LPN will perform tasks and responsibilities within the framework of health teaching and provisionof supportive and restorative care. License Practical Nurse (LPN) Job Responsibilities:Performs on site visits and consultations withclientsand staff todeterminecurrent health status and any intervention education and training toclientsand staff asindicatedby need onhealth health monitoring and screening toclients including phlebotomy procedures for which LPN is medical crisis intervention as neededinconsultation withCommunity Mental Health Nurse Medical Director or Medical Directors adequate provision and documentation of health services provided at the clinic providing medication refills prior authorizations and tasks related to ensuring prescribed medication regimens are made available to clients per Agency informationregardingclients medical health information from approved healthcare providersConsults withfamilies asappropriate toensure the communication of pertinent health-related for reviews as directed byCommunity Mental Health and transfer of health information with healthproviderand other service and updates required records and documents for services health care and other health related issues as emergency situations which may require flexibility of time and areas of other dutiesresponsibilitiesand related tasks as Practical Nurse (LPN) Qualifications:Graduate of an accredited LPN program with current licensure in NYS. One year of clinical experience as an LPN with experience working with mental health population preferred. Must have eligibility for full and unconditional participation in the Medicaid and Medicare programs. Excellent verbal and written communication skills. Ability to effectively use an electronic medical record for all documentation. Must have a valid New York State drivers license and access to reliable transportation when required.
