Licensed Practical Nurse (LPNLVN)

ATC Healthcare Services LLC of Nassau County has an IMMEDIATE NEED for an experienced Licensed Practical Nurse (LPN) to serve as a Treatment Coordinator in a Skilled Nursing and Rehabilitation Facility located in Freeport NY.

This is an excellent opportunity for a dedicated LPN with strong clinical organizational and communication skills to play a key role in coordinating patient treatments ensuring quality care and supporting positive patient outcomes in a fast-paced rehabilitation environment.

Schedule: Saturdays & Sundays 7:00 AM 3:00 PM

• Coordinate and implement patient treatment plans as directed by physicians and Registered Nurses.

• Administer medications treatments and nursing interventions in accordance with physician orders and facility policies.

• Monitor patient conditions and promptly report changes to physicians Registered Nurses and other healthcare professionals.

• Perform routine assessments and document patient responses to treatment.

• Review and update treatment records care plans and medical documentation.

• Schedule and coordinate appointments consultations diagnostic testing and follow-up care.

• Communicate treatment goals and patient progress with residents families and healthcare team members.

• Assist with wound care dressing changes catheter care and other clinical procedures as assigned.

• Ensure compliance with infection control standards patient safety protocols and regulatory requirements.

• Participate in interdisciplinary care conferences and contribute to care planning discussions.

• Educate patients and families regarding treatment plans medications and health management strategies.

• Maintain confidentiality of patient information in accordance with HIPAA regulations.

• Assist with quality assurance and performance improvement initiatives.

• Active New York State Licensed Practical Nurse (LPN) License required.

• Current CPR/BLS Certification required.

• Minimum one (1) year of nursing experience preferred.

• Experience in long-term care rehabilitation home care outpatient or clinical settings preferred.

• Knowledge of medical terminology treatment protocols and nursing practices.

• Proficiency with Electronic Medical Records (EMR/EHR) systems.

• Strong organizational communication and interpersonal skills.

• Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.

• Excellent critical thinking and problem-solving skills.
