ATC Healthcare Services LLC of Nassau County has an IMMEDIATE NEED for an experienced Licensed Practical Nurse (LPN) to serve as a Treatment Coordinator in a Skilled Nursing and Rehabilitation Facility located in Freeport NY.
This is an excellent opportunity for a dedicated LPN with strong clinical organizational and communication skills to play a key role in coordinating patient treatments ensuring quality care and supporting positive patient outcomes in a fast-paced rehabilitation environment.
Schedule: Saturdays & Sundays 7:00 AM 3:00 PM
Coordinate and implement patient treatment plans as directed by physicians and Registered Nurses.
Administer medications treatments and nursing interventions in accordance with physician orders and facility policies.
Monitor patient conditions and promptly report changes to physicians Registered Nurses and other healthcare professionals.
Perform routine assessments and document patient responses to treatment.
Review and update treatment records care plans and medical documentation.
Schedule and coordinate appointments consultations diagnostic testing and follow-up care.
Communicate treatment goals and patient progress with residents families and healthcare team members.
Assist with wound care dressing changes catheter care and other clinical procedures as assigned.
Ensure compliance with infection control standards patient safety protocols and regulatory requirements.
Participate in interdisciplinary care conferences and contribute to care planning discussions.
Educate patients and families regarding treatment plans medications and health management strategies.
Maintain confidentiality of patient information in accordance with HIPAA regulations.
Assist with quality assurance and performance improvement initiatives.
Active New York State Licensed Practical Nurse (LPN) License required.
Current CPR/BLS Certification required.
Minimum one (1) year of nursing experience preferred.
Experience in long-term care rehabilitation home care outpatient or clinical settings preferred.
Knowledge of medical terminology treatment protocols and nursing practices.
Proficiency with Electronic Medical Records (EMR/EHR) systems.
Strong organizational communication and interpersonal skills.
Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.
Excellent critical thinking and problem-solving skills.
