Schedule: 6:00 AM 2:30 PM (Sunday Saturday availability)

Job Summary

Navitas Healthcare LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN / T-LPN) for a short-term contract assignment in a supportive long-term care/assisted living environment. The ideal candidate will deliver high-quality patient care support medication administration and collaborate with the healthcare team to ensure optimal resident outcomes in a professional patient-centered setting.

Key Responsibilities
  • Provide direct nursing care under the supervision of an RN or physician.
  • Administer medications and treatments as prescribed and monitor patient responses.
  • Assess observe and document patient conditions vital signs and daily progress.
  • Assist residents with activities of daily living (ADLs) including hygiene mobility and comfort care.
  • Maintain accurate and timely clinical documentation in accordance with facility policies.
  • Communicate effectively with patients families physicians and interdisciplinary team members.
  • Report any changes in patient condition promptly to supervising staff.
  • Ensure a clean safe and organized care environment at all times.
  • Follow infection control protocols safety procedures and HIPAA compliance standards.
  • Perform additional nursing duties as assigned.
Qualifications
  • Active and unrestricted LPN / T-LPN license required.
  • Current BLS/CPR certification preferred.
  • Prior experience in assisted living long-term care rehabilitation or skilled nursing strongly preferred.
  • Strong clinical judgment communication and organizational skills.
  • Ability to work independently and collaboratively within a healthcare team.
  • Compassionate patient-focused approach to care.
  • Flexibility to work scheduled shifts including weekends if required.
