Mental Health and/or ID/DD experience is preferred for this position.

Responsibilities and Duties for LPN:

Implementing and maintaining Individual Service Plans.
Conducting ongoing physical and mental assessments.
Maintaining accurate consumer records monitoring consumer safety.
Supervising and monitoring individual medical treatment programs.
Administration and delivery of consumers prescribed medication as needed.
Monitoring and education regarding medication compliance and side effects.
Assessing consumers vital signs height weight medication compliance possible side effects and obtaining AIMS score or laboratory tests as ordered by a physician.
Completing PA (prior authorizations) and patient assistance for consumers psychiatric medication needs.
Compliance with agency policies and procedures and adherence to all relevant local state and federal legal and regulatory requirements.

Qualifications and Skills for LPN:

Graduation from an accredited Licensed Practical Nurse Program.
Current Virginia unrestricted LPN License or Current Unrestricted LPN License from a Nurse Licensure Compact State.
1-year LPN work experience in a mental health program and/or residential setting.
1-Year LPN work experience involving individuals with intellectual disabilities.
Valid drivers license with acceptable DMV record.
Proficiency in Microsoft Office.
Preferred: Experience using Electronic medical records; basic knowledge of Medicaid/Medicare Regulations.
