- Valid New York State LPN license required
- Must be authorized to work in the U.S.
- Reliable professional and patient-centered
- Must maintain professionalism in all written and verbal communications
- Must have updated medicals/vaccinations or be willing to obtain them
- Prior home care experience strongly preferred
- High-acuity home care experience preferred
- Drivers preferred
- Completed drug screen or completed Habituation Form
- Must successfully complete the Alvita Care onboarding process before accepting cases
- Competitive pay: $40$45/hour based on experience and assignment

* Subject to one year of employment and 1560 hours worked (average of 30 hours/week).
