Position Overview

As a Home Health LPN you will provide skilled nursing care to patients in their homes while collaborating with the interdisciplinary care team to ensure high-quality individualized care.

Youll work independently in the field - but never alone. Our Clinical Supervisors and operational staff are readily available to support you.

Responsibilities

• Deliver skilled nursing care in accordance with the established plan of care

• Monitor and assess patient conditions during home visits or shifts

• Communicate changes in patient status to the Clinical Supervisor

• Provide patient and caregiver education regarding medications treatments and disease management

• Accurately document patient care and clinical observations

• Coordinate care with other healthcare team members as appropriate

• Maintain compliance with agency and state guidelines

Schedule Options

• Day evening overnight shifts available

• Weekday and weekend opportunities

• Full-Time Part-Time PRN Per Diem

Compensation

$30.00 - $35.00 per hour
Pay rate based on experience case complexity and assignment type.

Qualifications

• Active LPN license (Pennsylvania and/or Compact License)

• Ability to work independently in a home setting

• Strong communication and organizational skills

• Reliable transportation
