Contract Type: Long Term Care Assignment
Shift: Day Shift 3x12 Hour Shifts
Shift Hours: 6:00 AM 6:00 PM
Guaranteed Hours: 36 Hours/Week
Weekend Requirement: Every Other Weekend

Navitas Healthcare LLC is seeking an experienced Licensed Practical Nurse (LPN) for a Long Term Care (LTC) assignment in Gifford Illinois. This is an immediate opening for a skilled nursing professional who is comfortable working in a fast-paced patient care environment and providing compassionate care to residents in a long-term care setting.

• Provide direct patient care in a Long Term Care environment
• Administer medications and treatments as prescribed
• Monitor assess and document patient conditions accurately
• Collaborate with nurses physicians and interdisciplinary healthcare teams
• Maintain timely and accurate charting using PCC documentation systems
• Support resident safety comfort and quality of care initiatives

• Active LPN License
• Previous Long Term Care (LTC) or Nursing Home experience preferred
• Experience with PCC charting systems is a plus
• Strong communication and patient care skills
• Ability to work scheduled weekends as required
