Job Summary

Navitas Healthcare LLC is seeking a Licensed Practical Nurse (LPN) for a long-term care assignment in Falmouth MA. The LPN will provide direct resident care supervise nursing assistants administer medications and treatments and ensure high-quality patient care in accordance with nursing standards and facility guidelines.

Key Responsibilities
  • Supervise nursing assistants and coordinate daily resident care activities

  • Administer medications and treatments as ordered by physicians

  • Maintain accurate charting documentation and resident records

  • Monitor resident conditions and communicate updates to physicians and nursing leadership

  • Assist with admissions transfers and discharges

  • Perform nursing procedures including catheterization wound care tube feedings suctioning and specimen collection

  • Take and document vital signs

  • Participate in resident care planning and orientation of residents/families

  • Ensure compliance with infection control and safety standards

  • Provide patient and family education

  • Maintain confidentiality and HIPAA compliance

  • Participate in facility inspections and surveys as required

Required Qualifications
  • Graduate of an approved LPN program or accredited nursing program

  • Active unrestricted Massachusetts LPN License required

  • Current BLS / CPR / AED certification required

  • Strong clinical communication and documentation skills

  • Knowledge of long-term care regulations and nursing practices

  • Ability to supervise and work effectively with staff members

  • Ability to lift up to 50 lbs and meet physical job requirements

  • Must be able to read write and communicate effectively in English
