The LPN will provide direct patient/resident care under the supervision of clinical leadership and support daily nursing operations including:

Administering medications and treatments as ordered
Monitoring patient condition and reporting changes
Documenting care accurately and timely
Supporting admissions discharges and transfers
Following infection control safety and facility protocols
Communicating professionally with patients families and care teams

Active LPN license in Pennsylvania or eligibility to work in PA
Recent LPN experience preferred
Experience in LTC SNF rehab post-acute or nursing home care is a plus
Strong bedside care documentation and communication skills
Reliable professional and assignment-ready
