Job Summary

Title: Licensed Practical Nurse (LPN)
Duration: 79.4 weeks
Location: Phoenix (AZ)

Schedule: Day Shift ( 6:30 AM 6:00 PM)
12-Hour Shifts 40 Hours Per Week

Position Overview:We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide exceptional care in a long-term care/skilled nursing environment. This is an excellent opportunity for an experienced nurse who is passionate about resident-centered care and thrives in a collaborative healthcare setting.

Responsibilities

  • Administer medications and treatments as prescribed and within scope of practice
  • Perform nursing procedures including wound care catheter care tracheostomy care gastrostomy/jejunostomy care and other assigned treatments
  • Monitor residents conditions and report responses to medications and treatments
  • Complete admission assessments for newly admitted residents
  • Assist with MDS assessments quarterly reviews and care plan development
  • Maintain accurate and timely clinical documentation
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives
  • Communicate effectively with residents families physicians and interdisciplinary team members
  • Provide detailed shift reports and ensure continuity of care
  • Attend staff meetings and ongoing training programs

Qualifications

  • Active Licensed Practical Nurse (LPN) license in good standing
  • Experience in Long-Term Care (LTC) Skilled Nursing Facility (SNF) Rehabilitation or Geriatric Nursing preferred
  • Knowledge of medication administration and nursing best practices
  • Experience with MDS assessments and care planning preferred
  • Strong assessment critical thinking and communication skills
  • Ability to respond effectively to medical emergencies and changing resident conditions
