Job Summary

Licensed Practical Nurse (LPN) Long-Term Care

Location: Falmouth MA
Duration: 13 Weeks
Shift: 8-Hour Shifts (Days & Evenings)
Schedule: 7:00 AM 3:30 PM 3:00 PM 11:30 PM
Pay Rate: $1900 $2150/Week (DOE)

Position Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide direct nursing care in a Long-Term Care setting. The ideal candidate will have strong clinical skills experience in resident-centered care medication administration and the ability to work collaboratively with residents families and interdisciplinary healthcare teams.

Responsibilities
  • Provide direct nursing care according to individualized care plans and physician orders.

  • Administer medications treatments and therapeutic interventions safely and accurately.

  • Perform wound care catheter care tube feedings specimen collection and other nursing procedures.

  • Monitor resident conditions and report changes to physicians and care teams.

  • Record vital signs and maintain accurate clinical documentation.

  • Implement physician orders and update resident records.

  • Supervise Nursing Assistants and coordinate resident care activities.

  • Educate residents and families regarding treatments and ongoing care needs.

  • Ensure compliance with LTC regulations and facility policies.

  • Maintain a safe and therapeutic care environment.

Requirements
  • Graduate of an accredited LPN program.

  • Active Massachusetts LPN License.

  • Current BLS Certification.

  • CPR and AED Certification.

  • LTC SNF Rehab or Geriatric Nursing experience preferred.

  • Experience supervising Nursing Assistants preferred.

  • Strong clinical documentation and communication skills.

  • Ability to work every other weekend and rotating holidays.
