Associates in a related field preferred. Must have 2 years of clinical experience providing nursing services to patients from diverse economic and cultural backgrounds as well as exceptional communication interpersonal and organizational skills are required. Outpatient primary care settings experience preferred. Bi-lingual English/Spanish a plus. Demonstrates effective teamwork by interacting in a positive manner with colleagues visitors and patients caregivers and families. Contributes to a collaborative work environment by initiating open communication a positive intent and offering assistance to support patients families and care to a safe and secure work environment for patients visitors and colleagues by following established procedures and patient and family cultural spiritual and personal norms by acknowledging the essence of each person and appreciating and responding to the uniqueness of each chronic care disease support to the patients by phone in person and in all aspects of the care treatment and services provided and in accordance with the clinics integrated model of care. Develops patient focused self-management goals and care plans with the gaps in care for screenings chronic care tests (such as HgbA1c for Diabetics) health literacy assessments tobacco screenings BMI etc. are completed in timelines established by best and provides input for patients in multi-disciplinary team meetings weekly daily multi-disciplinary strong nursing knowledge base and skill in job responsibility and accountability for coordinating care in collaboration with the medical that continuity of care is maintained in all aspects of patient to medical staffs effectiveness by identifying patient care issues that must be addressed; providing information; recommending options and courses of action; and implementing medical providers as liaison to medical patients and employees by adhering to infection-control policies and timely and accurately patient care services in the electronic health record. Tracks referrals labs and diagnostics in accordance with policy in the electronic health record. Must be able to pass a respirator fit test at time of hire and on an annual with regular staff meetings focused on coordinating care within an interdisciplinary job responsibilities consistent with standards for best practice nursing including excellent communication with all providers to ensure the delivery of high quality to develop knowledge and understanding about the history traditions values family systems and artistic expression of groups served as well as uses appropriate methodological approaches skills and techniques that reflect an understanding of culture.#IND123 #LI-MM1
