Licensed Practical Nurse (LPN)
Location: Grand Junction CO
Type: Contract (13 Weeks Possible Extension)
Pay: $40/hr $49/hr
Shift & Schedule: Night Shift 7:00 PM 7:00 AM 36 Hours/Week
We are seeking an experienced Licensed Practical Nurse (LPN) to provide compassionate resident-centered care in a Long-Term Care and Skilled Nursing setting. The LPN will work closely with residents families and the interdisciplinary healthcare team to ensure quality patient care and support resident well-being.
Provide direct nursing care to residents in a long-term care and skilled nursing environment.
Administer medications treatments and nursing interventions as prescribed.
Monitor residents conditions and report changes to the healthcare team.
Assist with admissions assessments care planning and documentation.
Support residents with rehabilitation and recovery goals.
Perform wound care basic treatments and other nursing procedures within scope of practice.
Maintain accurate and timely documentation in the electronic medical record system.
Collaborate with physicians RNs CNAs therapists and other healthcare professionals.
Promote resident safety dignity comfort and quality of life.
Educate residents and families regarding care plans and treatment goals.
Follow facility policies infection control guidelines and regulatory requirements.
Active LPN License.
Current BLS/BCLS Certification.
Minimum 1 year of LPN experience required.
Previous experience in Long-Term Care (LTC) Skilled Nursing (SNF) Rehabilitation or Geriatric Care preferred.
Experience with EMR systems preferred.
Strong communication documentation and patient care skills.
Long-Term Care or Skilled Nursing Facility experience.
Medication administration experience.
Patient assessment and monitoring skills.
Strong teamwork and organizational abilities.
