We are seeking a dedicated Permanent Licensed Practical Nurse (LPN) to provide high-quality nursing care in a long-term care setting. The LPN will be responsible for delivering compassionate patient-centered care while supporting residents daily clinical and personal needs.

This role requires strong clinical judgment consistency and the ability to work effectively within an interdisciplinary care team in a structured care environment.

• Provide direct nursing care in accordance with established care plans and physician orders
• Administer medications and treatments accurately and safely
• Monitor document and report changes in resident condition
• Assist with admissions discharges and updates to care plans
• Collaborate with RNs physicians and interdisciplinary care teams
• Maintain accurate and timely clinical documentation
• Follow infection control and safety procedures
• Support resident comfort dignity and quality of life
• Participate in care planning and team communication

• Active Licensed Practical Nurse (LPN) license in the state of practice
• Completion of an accredited Practical Nursing program
• Current CPR/BLS certification preferred or required per facility
• Strong communication and teamwork skills
• Ability to work long day shifts (06:45 AM 07:19 PM)
• Prior long-term care experience preferred
