About the Role
We are seeking an experienced Licensed Practical Nurse for a 13-week contract opportunity in Roseau Minnesota. This clinic-based role supports patient-centered care patient assessments education documentation and collaboration with the multidisciplinary care team.

Responsibilities

• Perform and document patient assessments within LPN scope of practice.

• Assist with patient care planning and implementation.

• Monitor patient condition and report changes appropriately.

• Support health history admission discharge planning and patient education.

• Provide patient care and personal care support as needed.

• Communicate patient progress with nurses providers patients and families.

• Document nursing care accurately and timely using EPIC.

• Collaborate with the multidisciplinary team to support continuity of care.

Requirements

• Active Minnesota LPN license required.

• Minimum 2 years of LPN experience required.

• Clinic experience preferred.

• EPIC experience preferred.

• Day shift availability required.

• Strong communication documentation and patient care skills required.

• Ability to work effectively as part of a multidisciplinary team required.
