About the Role We are seeking an experienced Licensed Practical Nurse for a 13-week contract opportunity in Roseau Minnesota. This clinic-based role supports patient-centered care patient assessments education documentation and collaboration with the multidisciplinary care team.
Responsibilities
Perform and document patient assessments within LPN scope of practice.
Assist with patient care planning and implementation.
Monitor patient condition and report changes appropriately.
Support health history admission discharge planning and patient education.
Provide patient care and personal care support as needed.
Communicate patient progress with nurses providers patients and families.
Document nursing care accurately and timely using EPIC.
Collaborate with the multidisciplinary team to support continuity of care.
Requirements
Active Minnesota LPN license required.
Minimum 2 years of LPN experience required.
Clinic experience preferred.
EPIC experience preferred.
Day shift availability required.
Strong communication documentation and patient care skills required.
Ability to work effectively as part of a multidisciplinary team required.
