Job Title: Licensed Practical Nurse-LPN
Job Location: Clarks Summit PA;Scott Township PA and Nicholson PA
Immediate Need:
Clarks Summit PA - All SHIFTS AVAILABLE for a 24-HR a day case
Scott Township PA - Monday through Friday 7:15am to 4:45pm
Nicholson PA - Monday through Friday 8:20am to 2:45pm
Factoryville PA- 7 days a week/ 16 hours a day availability
Rates: $28.00-$32.00

Job responsibilities include but not limited to the following:

• One to One patient care in home
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion teaching and training of family members
