Position Overview
As a Licensed Practical Nurse (LPN) you will play a key role in delivering high-quality resident-centered care at an Assisted Living and Memory Care community. Youll collaborate closely with caregivers leadership and healthcare providers to ensure safe compassionate and personalized care.
Key Responsibilities
- Provide direct nursing care including medication administration treatments and assessments
- Monitor resident health and communicate changes to the care team
- Assist in developing and updating individualized care plans
- Support residents with memory loss through patience empathy and engagement
- Build strong relationships with residents families and team members
- Maintain accurate and timely clinical documentation
- Mentor and support caregivers and new staff
- Ensure compliance with state regulations and company policies
Qualifications
Required:
- Graduate of an accredited practical nursing program
- Active Minnesota LPN license in good standing
Preferred (not required):
- Assisted Living Memory Care or Long-Term Care experience
