As a Home Health Licensed Master Social Worker youll help patients and families navigate lifes challenges by connecting them with resources providing emotional support assisting with care planning and helping remove barriers that impact health and well-being.

• Complete psychosocial assessments for home health patients
• Identify social emotional environmental and financial needs
• Connect patients and families with community resources and support services
• Assist with long-term planning and discharge planning
• Provide counseling education and emotional support to patients and caregivers
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate documentation

• Active Virginia Licensed Master Social Worker (LMSW) License Required
• Masters Degree in Social Work from an Accredited Program
• Valid Drivers License and Reliable Transportation
• Strong Communication Assessment and Documentation Skills
• Home Health or Healthcare Experience Preferred but Not Required
• Passion for Helping Patients and Families Overcome Challenges
