As a Home Health Licensed Master Social Worker youll provide compassionate guidance advocacy and emotional support while helping patients overcome barriers that affect their health safety and quality of life.

Your work helps patients remain independent strengthens families and ensures no one has to face lifes challenges alone.

• Complete comprehensive psychosocial assessments
• Identify emotional social environmental and financial barriers to care
• Connect patients and families with community resources and support services
• Assist with discharge planning and long-term care planning
• Provide counseling education crisis intervention and emotional support
• Collaborate with physicians and interdisciplinary team members
• Advocate for patients while coordinating appropriate community services
• Complete timely and accurate clinical documentation

• Active Virginia Licensed Master Social Worker (LMSW) License Required
• Masters Degree in Social Work from an Accredited Program
• Valid Drivers License and Reliable Transportation
• Strong Assessment Communication and Documentation Skills
• Home Health or Healthcare Experience Preferred but Not Required
• Compassion for Serving Diverse Patient Populations
