Job Summary

Licensed Master Social Worker (LMSW)

Location: New York NY 10016

Duration: 14 Weeks Contract

Shift: Day Shift

Hours: 9:00 AM 5:00 PM MondayFriday

We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide psychosocial support discharge planning and care coordination services within a hospital setting. This role involves working closely with interdisciplinary teams to ensure patients receive comprehensive care and appropriate post-discharge resources.

Responsibilities:
Conduct comprehensive psychosocial assessments for patients and families
Provide supportive counseling and therapeutic interventions
Identify emotional social environmental and financial factors impacting patient care
Assist patients and families in coping with illness hospitalization and recovery
Develop and implement discharge plans in collaboration with the healthcare team
Coordinate referrals to community agencies rehabilitation services behavioral health resources and other support programs
Facilitate safe and effective transitions of care
Advocate for patient needs and appropriate support services
Collaborate with physicians nurses case managers and interdisciplinary teams
Maintain accurate and timely documentation within the medical record
Ensure compliance with hospital policies regulatory standards and social work ethics

Requirements:
Masters Degree in Social Work required
Minimum 1 year LMSW experience required
Hospital experience required
Valid Licensed Master Social Worker (LMSW) license issued by NYSED required
Strong psychosocial assessment and counseling skills required
Experience with hospital discharge planning and care coordination required
Excellent communication and interpersonal abilities required
Ability to work effectively in a fast-paced healthcare environment required
