Job Summary

Pay: $35.48 - $56.02/hour DOE
Full-Time Permanent Position Monday-Friday Schedule
Social workers must provide direct support and case management services to assigned patients and families
Coordinate resources and services to support positive patient outcomes
Advocate for patients while helping navigate social emotional and community-based needs
Current Licensed Social Worker (LSW) license required
LICSW candidates are highly preferred
Hospital or healthcare experience preferred
