Easterseals Florida is looking to fill our Lead Community Health Resources Navigator (for children and adults with disabilities) position serving in Mascotte Minneola Groveland and 4 Corners (Hartwood Marsh Road to the end of South Lake County) areas. The Lead Community Health Resources Navigator works with families with children and adults with disabilities in need of a wide variety of assistance for whole health. The position is responsible for leading the team in providing health information and advocacy for children/adults and their caregivers making referrals to community resources and conducting disability screenings for children ages five and younger. The Lead Community Health Resource Navigator is also responsible for recruiting clients. This position works extensively with community partners to identify clients and to ensure access to needed health and disability services. The qualified candidate will be a current or former resident of Lake County Florida.

A snapshot of what youll do:

  • Coordinate and collaborate with South Lake Countys neighborhood services as well as local community programs childcare centers and schools to obtain referrals for children and adults with disabilities and their families in need of guidance and support to secure health and disability services
  • Collaborate with community partners to coordinate vendors and support the planning and executiion of community events
  • Successfully integrate and participate in community events and committee membership within South Lake County and with community partners
  • Administer developmental screenings for children ages 05 using the Ages & Stages Questionnaire (ASQ) to monitor progress and support early identification of developmental needs.
  • Prepare quarterly and annual reports develop success stories and track program data
  • Provide guidance mentorship and ongoing support for the team to foster a collaborative environment and ensure alignment with program goals deadlines and quality standards
  • Participates in the hiring process including candidate evaluation provide training and helps track team progress to ensure performance expectations and timelines are consistentantly met

Qualifications:

  • Bilingual (Spanish/English) preferred
  • Bachelors degree in social work education or related field and four years of experience
  • Certification in Health Services (CNA LPN etc.) also considered
  • Experience/knowledge of the South Lake County (Mascotte Minneola Groveland and 4 Corners) social service network
  • Experience working with individuals with developmental disabilities a plus
  • Ability to work independently and effectively with minimal on-site supervision
  • Ability to meet our approved driver qualifications in order to drive your personal vehicle as required for program-related needs (minimum age of 21 and good driving history)
