Role: LEAD Case ManagerReports to: Assistant Director LEADProgram: 1602-DHS ODRDepartment: OutreachLocation: Campion - 1447 16th Street Santa Monica CA 90404Setting: 100% OnsiteSchedule: Monday - Friday 8:00am - 4:30pmStatus:Full-time/Non-Exempt/Non-ManagementBenefits:Medical Vision Dental Life Insurance 403(b) Retirement plan Employee Assistance Program (EAP) : 1Summary:With the support of and in partnership with the LEAD team Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals and is responsible for identifying engaging and providing support to homeless clients referred from the LAPD Pacific division including aftercare services and follow up support once housed. The goal of LEAD is to connect with clients experiencing frequent contacts with police due to their homeless status and enroll them in services to prevent entry to the jail and criminal justice system. The Case Manager provides information and support for program members and assists members in linking to a variety of resources and services as needed. The Senior Case Manager will model competence in wellness recovery and coping and is responsible for maintaining the highest quality services and efficiency of Duties and Responsibilities:Identify engage and provide support to clients in the LEAD program living on the streets in Pacific Division targeting those clients who are vulnerable due to medical psychiatric or addiction in collaboration with other team members to remove barriers to permanent housing; e.g. acquisition of identification legal aid benefits assistance life skills acquisitionWork collaboratively as a team to identify appropriate services taking into account each individuals physical and cognitive needsProvide case management and aftercare/follow-up services to clients placed in permanent housingReport all statistics and updates for reporting purposesProvide initial contact needs assessment information education and referrals to the homelessProvide case management basic services and advocacy for clientsRefer target clientele to appropriate services (e.g. health mental health interim housing etc.)Develop effective trusting relationships with clients that facilitate provision of servicesWork in collaboration with LEAD ODR team members and outside agency staff to connect clients to appropriate servicesMaintain and record accurate electronic records client files and statistical informationAttend all staff meetings trainings and retreatsParticipate in OWG and other community meetings as :High School Diploma or GED or EquivalentValid CA Drivers license reliable car auto insurance and an acceptable driving and able to transport Clients in your personal knowledge of and experience with the issues faced by the population served (homelessness mental illness substance abuse medical issues and domestic violence)Willingness to work in a high tolerance Harm Reduction model with clients who have multiple barriersWorking knowledge of LA County agencies serving the homelessExcellent human relations/communication skills; ability to act in professional manner regardless of circumstancesSkilled in non-violent crisis interventionAbility to self-motivate and organize time effectively and efficientlyComputer literate; able to use computerized database system for information managementPassion for working with the population servedAbility to work as part of an interdisciplinary teamPreferred QualificationsBachelors degree; or a minimum two (2) years experience working with homeless individuals preferably those living with mental illness and/or substance addictions or an equivalent combination of education and Environment:Combination of field and office required to sit stand bend; occasionally lift or carry up to 35 occasion walk or drive to different local necessitate working in busy and loud be exposed to elements like cold heat dust noise and need to bend stoop twist and sit throughout the day.
