Lead Care Manager Field-Based Position

Location: Monterey/South Monterey County CA
Salary: $28.00$35.00/hour (based on experience)
Travel Requirement: Approximately 90% travel throughout assigned county

About This Role

We are seeking a compassionate and experienced Lead Care Manager to support Medi-Cal members through an Enhanced Care Management program. You will work directly in the community helping members navigate healthcare and social services while addressing barriers related to homelessness chronic health conditions behavioral health challenges and other complex needs.

What Youll Do
  • Conduct member outreach assessments and eligibility reviews
  • Develop individualized care plans based on member needs and goals
  • Conduct regular face-to-face visits in members homes and communities
  • Maintain contact via in-person visits phone and telehealth
  • Coordinate services with healthcare providers and community organizations
  • Accompany members to medical appointments when needed
  • Connect members with community resources (housing food assistance transportation financial support)
  • Track referrals and ensure members receive necessary services
  • Identify safety and behavioral health concerns and develop crisis plans
  • Document activities and progress notes in electronic health records
What Were Looking For
Required:
  • High school diploma or equivalent
  • Medical Assistant diploma OR Certified Nursing Assistant (CNA) credential
  • Minimum 2 years of professional healthcare experience
  • Valid drivers license with clean driving record
  • Current automobile insurance and reliable personal transportation
  • Proficiency with Microsoft Office email and electronic documentation
  • Ability to manage a field-based caseload independently
Highly Preferred:
  • Experience with Enhanced Care Management CalAIM or Medi Cal programs
  • Background working with vulnerable or underserved populations
  • Knowledge of community resources and social determinants of health
  • Bilingual English and Spanish capabilities
What to Expect

This position requires extensive fieldworkconducting home and community visits frequent walking and standing occasional driving and lifting supplies up to 20 pounds. Youll work in diverse settings including residences healthcare facilities community organizations and outdoor environments.
