Position Overview:
Juvenile Forensic Service Coordinators provide specialized service coordination to25-30youth(ages 10 18)involved with Juvenile Justice Coordinators completeassessmentsindividualizedserviceplan andcrisis plan treatmentreferrals and connectionsto community resourcesto address social determinants of closely collaborate with Juvenile Justice System to supportclients withmeeting the expectations of the service coordinatorsbuild trusting relationshipswith youth and familiesmaintaintimelydocumentation meet programutilization coordinatemultidisciplinary care and achieveservice plan goals.
Key Responsibilities:
Client Support and Case Management
Serve as theSingle Point of Accountabilityfor clients and families acting as the primary contact and advocate for services.
Build andmaintainenduring relationshipswith clientsand their familiesthrough persistent outreach and engagement.
Developcomprehensive service planstailored to individual strengths and needs regularly updatingandpresentingprogressto thecourt.
Support clients duringtimes of transition including those preparing for return to community from treatment programs detention facilities and hospitalization.
Coordinate careacross various providers including behavioral healthjuvenile probationchild welfaredrug and alcohol servicesschools and community resources.
Maintainmonthly face-to-face contactwith clients ensuring services are provided in line with program withclients throughout Allegheny County including homes schools treatment programscourts juvenile probation placements detention centers county jail etc.
Collaboration and Advocacy
Work closely with familiesmental health servicesprobation officers educational staff and community providers to addressbarrierstoservice plan goals.
Facilitateinterdisciplinary team meetingsensuringfollow-through onaction stepsand ensuring continuity of care.
Act as anadvocatefor clients helping remove systemic barriersresolving issues and fostering full inclusion within their community.
Attend school-based meetings and offereducational advocacyas needed ensuring clients have the support they need to thrive.
Documentation and Compliance
Completeassessments service plans and case noteswithin specifiedtimeframes ensuring all documentationcomplies withstate and agency regulations.
Maintainaccuraterecordsof all interactions including family meetings service linkages and follow-up activities in electronic health records.
Meetmonthlyutilizationexpectations.
Comply withHIPAA confidentiality policiesandparticipatein regular audits and program evaluations.
Professional Development
Participate inongoing trainingand development opportunities staying updated on best practices and trends inbehavioralhealth juvenile justiceeducationand service coordination.
Activelyutilizeindividual and group supervisionto triage clientsmonitortrends enhancethe program.
Required Skills and Qualifications:
Bachelorsdegreein Social Work Psychology Education Criminal JusticePolitical Scienceor a related field (Mastersdegreewelcome).
Twoyears of experienceina behavioralhealth juvenile justiceorhuman servicesetting.
Strongorganizational skillswith the ability to manage multiple priorities and meet deadlines.
Excellentcommunication and interpersonal skills with the ability to work independently and as part of a multidisciplinary team.
Demonstrated cultural competence and the ability to work withdiverse populations.
Additional Requirements:
ValidPennsylvania drivers licensewith access to a reliable vehicle.
Currentauto insurancethat meetsagencyrequirements.
Act 33 Act 34FBI ClearancesInstitutional Clearances.
Ability to completerequiredonboardingtrainingwithin30 daysof hire.
WorkEnvironmentand Benefits:
Full-time position(40hoursper week) with occasional evening or weekend hours to meet client needs.
