The Illinois Department of Veteran Affairs is seeking to hire an independent highly organized and detail-oriented Health Information Associate for the Chicago VA home located in Chicago Illinois. This position will be responsible for a variety of paraprofessional health information functions such as assisting in planning organizing and maintaining statistical records reports and a variety of Health Information documentspertaining to residents answering telepones for medical informatio from other departments and reviewing health records for accuracy.
- Under general supervision performs a variety of paraprofessional health information functions. Assists inplanning organizing and maintaining statistical records reports and a variety of Health Information documentspertaining to residents at the Illinois Veterans Home.
- Assists in planning organizing and maintaining patient medical information received from a variety of healthcarefacilities (e.g. local health providers VA medical centers V.A. hospitals etc.) and processes said informationthrough established procedures.
- Collects and records information associated with residents upon admission death or discharge.
- Answers the telephone and handles request for medical information from other departments which requires theknowledge of medical psychological psychiatric and anatomical terminology.
- Implements the Authorization of Release Information requiring signatures and forwarding on to appropriateparties which requires the knowledge of federal and state statutes regarding confidentiality HIPAA and release ofinformation.
- Reviews the health information record for accuracy timeliness and completeness and to interpret healthinformation records.
- Performs other duties as required or assigned which are reasonably within the scope of the duties enumeratedabove.
- Requires knowledge skill and mental development equivalent to two years of college and one year of health related officeexperience or completion of high school and three years of health related office experience.

2. Requires ability to meet all agency vaccine/health-related policies and guidance.
3. Requires ability acquire and maintain Cardiopulmonary Resuscitation (CPR) certificate.
4. Requires the ability to utilize office equipment including personal computers.
5. Requires ability to attend seminars conferences and trainings to stay current on methods tools ideologies or otherindustry related topics relevant to the job duties.
6. This position is considered sedentary work as defined by the U.S. Department of Labor (20 CFR 404.1567(a)). Sedentarywork involves lifting no more than 10 pounds at a time and requires occasional lifting carrying walking and standing.
7. Requires ability to travel in the performance of duties with overnight stays as appropriate.
8. Overtime is a condition or employment and you may be required to work overtime including scheduled unscheduled orlast-minute overtime.
