Integrated Behavioral Health Counselor

Salary Range: $54700 $80300

We are seeking a compassionate and skilled Integrated Behavioral Health Counselor to join our collaborative healthcare team. This role is responsible for providing evidence-based behavioral healthcare services to diverse patient populations experiencing emotional behavioral and psychiatric concerns.

As part of an integrated behavioral health and primary care model the counselor will work closely with medical providers to deliver screenings assessments crisis intervention brief therapy care coordination case management and follow-up services. This position offers the opportunity to make a meaningful impact through collaborative patient-centered care across the lifespan.

• Provide initial behavioral health screenings assessments brief therapy crisis intervention psychoeducation case management and referral services within a primary care setting.
• Conduct suicide and homicide risk assessments using evidence-based screening and safety planning tools.
• Deliver crisis management services and coordinate referrals for higher levels of care when appropriate.
• Complete culturally responsive mental health evaluations psychosocial assessments and DSM-V-TR diagnostic screenings.
• Utilize evidence-based interventions and treatment approaches to support patient wellness and recovery.
• Develop individualized treatment plans with measurable patient-centered goals and intervention strategies.
• Collaborate with interdisciplinary medical and behavioral health teams to support whole-person care.
• Assist medical providers in identifying and addressing behavioral health concerns that impact patient outcomes treatment adherence and motivation.
• Participate in clinical team meetings case consultations quality assurance initiatives and care coordination activities.
• Maintain timely and accurate documentation in the Electronic Medical Record (EMR) system.
• Provide referrals and connect patients with community-based behavioral health resources and support services.
• Maintain active licensure and comply with all professional standards and continuing education requirements.
• Perform additional duties and special projects as assigned.

• Licensed Independent Clinical Social Worker (LICSW) or Licensed Professional Counselor (LPC)
• Minimum of three (3) years of experience in behavioral healthcare
• Ability to obtain credentialing with CMS and insurance providers
• Strong verbal and written communication skills

• Two (2) years of experience working in a medical or integrated healthcare setting
• Bilingual in Spanish preferred
• Experience with telehealth and integrated care models

• Knowledge of evidence-based counseling interventions and brief treatment modalities
• Understanding of crisis prevention and intervention including suicide and violence risk assessment
• Strong understanding of ethics confidentiality and legal responsibilities in behavioral healthcare
• Experience providing trauma-informed and culturally responsive care
• Ability to collaborate effectively with multidisciplinary teams and community partners
• Strong organizational and documentation skills
