• Work Schedule:P.m. shifts with an every other weekend rotation; P.m. shifts - 3:00pm - 11:30pm

• Responsible for case review and telephone communications with patients for assessment and assistance with appropriate level of care needs and/or safety planning if appropriate.
• Responsible for answering phone calls telephone assessment and triage and scheduling full patient assessments with appropriate level of care resource.
• Functions as a member of the multi-disciplinary treatment team.
• Responsible for communications across entire health system related to patient referrals and outcomes.

• Bachelors degree in Social Work Psychology Counseling or a related field required.

• Previous experience in mental health triaging required.
