Job Summary
Position Overview
The Intake & Scheduling Coordinator is the operational hub of Elevate Infusions day-to-day patient services. You will serve as the primary point of contact between our pharmacy partners patients and nursing staff ensuring referrals are accepted smoothly patients are onboarded with care nurses are scheduled efficiently and our pharmacy partners are kept in the loop every step of the way. This role is ideal for a detail-oriented people-first communicator who thrives in a fast-paced healthcare environment.
Core Responsibilities
Referral Intake
Receive and process inbound referrals from pharmacy partners and referral sources
Review referral documentation for completeness and follow up on any missing information
Enter and maintain accurate patient records in the agencys system of record
Communicate referral status and updates to pharmacy partners in a timely manner
Patient Intake & Onboarding
Conduct intake calls with new patients to gather demographic insurance and clinical information
Explain the home infusion nursing process and set clear expectations for the patient experience
Coordinate with the clinical team to ensure a smooth handoff from intake to first visit
Nurse Scheduling
Schedule home infusion nursing visits based on patient needs nurse availability and geography
Communicate schedules clearly to both patients and nursing staff
Proactively identify and resolve scheduling conflicts or coverage gaps
Adjust schedules as needed due to cancellations add-ons or urgent visit requests
Pharmacy Partner Communication
Serve as a primary liaison between Elevate Infusion and our specialty pharmacy partners
Communicate nurse availability visit confirmations and scheduling updates to pharmacy contacts
Relay clinically relevant updates and visit outcomes to the pharmacy as appropriate
Build and maintain strong working relationships with pharmacy partner teams
Administrative & Organizational Support
Maintain organized up-to-date records for all active patients and scheduled visits
Track referral-to-visit conversion and flag any delays or issues to leadership
Support the team with general administrative tasks as needed we are a startup and flexibility is key
Requirements
High school diploma or equivalent required; Associates or Bachelors degree a plus
13 years of experience in healthcare administration medical office coordination home health or a related field
Pharmacy technician background strongly welcomed
Strong organizational skills with the ability to manage multiple priorities simultaneously
Excellent verbal and written communication skills
Comfortable working in a fast-paced evolving startup environment
Proficiency with basic computer applications; experience with EMR or scheduling software a plus

Hourly Rate: $25.00/hour
