This is a union eligible SUMMARYHealthRIGHT 360 a nonprofit organization and a family of programs is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services including mental health care residential and outpatient substance use treatment and primary health services. Additionally we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health we empower individuals to overcome challenges by addressing social determinants of health fostering resilience and facilitating San Francisco Intake Services Program works with high-risk populations from a variety of backgrounds. Our programs are open and welcoming to adult participants of any gender identity. We accept the majority of participants who seek Outpatient treatment including walk-ins referrals from other agencies and step-down transitional clients from residential treatment Intake Coordinator I is an integral part of an interdisciplinary team that supports our clients through the assessment of their needs and strengths in support of their health and recovery. Intake Coordinator I will work in a collaborative environment to connect clients to the level of treatment they need while managing a small caseload of individual clients or facilitating group treatments. Since many of our clients have backgrounds in the criminal justice system are chronically homeless and/or otherwise economically disenfranchised Intake Coordinator I will show flexibility in being able to meet the changing needs of our programs can tolerate working in a fast-paced variable working environment and will assess our clients for harm to self and others and will diagnose and treat such clients with co-occurring RESPONSIBILITIESCustomer Service and Communication Responsibilities:Welcomes clients to healthRIGHT360 substance abuse mental health and gambling services and provides high quality customer face-to-face interviews with clients to screen for appropriate treatment and coordinates with outside referral agencies and stakeholders via phone fax email and in Entry and Compliance Responsibilities:Ensure all required admission paperwork is completed accurately and entered in a timely manner in multiple electronic data program placements based on client needs and bed client triage /waitlist and conducts daily overview of program and Assessments:Identify and evaluates an individuals strengths weaknesses problems and needs for the development of a treatment level of care assessments to help determine eligibility for withdrawal management and residential substance use disorder treatment the agencys assessment tools to gather client information for admissions directly from the mental health/level of care assessments and diagnoses for adults seeking residential regularly with the Director to ensure clients meet the criteria for safe admission to the appropriate level of residential crisis intervention and referrals to high-level of care as to San Francisco County Jail sites as needed to perform mental health/level of care assessments with inmates seeking residential treatment upon a small individual therapy caseload or facilitate group therapy thorough written documentation of all services provided in compliance with regulations contracts and behavioral health in efforts to solve issues even when faced with internal barriers; takes personal responsibility for customer service perform other duties as assigned. QUALIFICATIONSEducation Certification and Experience Drug and Alcohol Registration from an accredited credentialing body (CADTP CCAPP CAADE).Minimum of two years of experience in dual diagnosis substance abuse treatment in an inpatient or outpatient community mental health setting conducting mental health assessments and providing appropriate with mental health Medi-Cal clinical paperwork timely record-keeping abilities and dual entry in EHR a valid drivers license and is able to pass an agency driving test to use an agency vehicle for off-site and Alcohol Certification from an accredited credentialing body (CADTP CCAPP CAADE).Knowledge Familiarity with Drug-Medical and ASAM of understanding of diagnostic criteria for ICD-10/DSM V classification of mental disorders.
