Senior Doc is expanding our Inpatient Palliative Care team and is looking for a Nurse Practitioner or Physician Assistant with a passion for caring for patients with serious or chronic illness.

Position Overview:
We are seeking a compassionate and experienced Palliative Care Nurse Practitioner or Physician Assistant to provide patient-centered care focused on symptom management pain control and quality of life for individuals with serious or chronic conditions.
This position supports inpatient palliative care community-based care and home visits with flexibility to practice across settings based on patient needs and program demand.
Key Responsibilities:
Provide comprehensive palliative care consultations across inpatient community and home-based settings
Develop and manage inpatient consultation emphasizing comfort symptom control and patient goals
Collaborate closely with physicians nurses social workers chaplains and interdisciplinary care teams
Serve as a clinical leader within the interdisciplinary team
Educate patients and families on disease progression treatment options and goals of care
Demonstrate strong time management while managing a diverse patient caseload
Participate in interdisciplinary meetings case conferences and care planning
Qualifications:
Active California Nurse Practitioner or Physician Assistant license
Strong interdisciplinary team leadership skills
Excellent time management and organizational abilities
Comfortable with home visits and community driving
Strong communication skills with patients families and care teams
Ability to work independently while coordinating care across settings
Preferred / Plus Qualifications:
Credentialed at either Orange County LA County Long Beach Temecula Valley Riverside San Bernardino San Diego County area hospitals
