Join University Home Care Inc as a Hospice/Home Care Register Nurse inDearborn MI providing compassionate and skilled nursing care to patients in the comfort of their this role you will support patients and their families through end-of-life care and chronic illness management ensuring dignity comfort and quality of life.

• Provide skilled nursing care to hospice and home care patients according to established plans.
• Assess patient condition and effectively communicate changes to the care team.
• Develop implement and update individualized care plans in collaboration with interdisciplinary teams.
• Educate patients and family members regarding illness treatment options and self-care techniques.
• Administer medications and treatments as prescribed by physicians.
• Document all patient interactions and care activities accurately and timely.
• Coordinate with physicians social workers and other healthcare professionals to provide holistic care.

• Current Registered Nurse (RN) license in the state of Michigan.
• Experience in hospice or home care nursing preferred.
• Strong assessment communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Valid drivers license and reliable transportation.
• Compassionate attitude with a commitment to patient-centered care.
