- As an RN Liaison you will evaluate hospitalized patients for hospice eligibility and interest in hospice care educate patients and families about the benefits and support available through hospice services and facilitate a seamless transition to hospice care when appropriate.
- Assess patients for hospice eligibility and appropriateness of services
- Provide education to patients families and healthcare professionals regarding hospice philosophy services and benefits.
- Collaborate with hospital physicians and Hospice Medical Directors to confirm hospice eligibility.
- Coordinate and communicate key clinical information to ensure a smooth transition to the hospice admitting nurse.
- Work closely with hospital case managers discharge planners physicians and other healthcare professionals.
- Serve as a trusted resource and advocate for patients and families exploring end-of-life care options.
- A current license to practice as a Registered Nurse in the State of New York
- Associates Degree in Nursing BSN preferred
- 1 year of recent acute care RN experience required
- Hospice Palliative Care or RN Liaison experience a plus but not required.
- The RN must be able to communicate effectively verbally and written
- Valid Drivers License and Reliable vehicle
- High level of interpersonal skills to establish and maintain relationships with patients families physicians and coworkers
- Understanding of Hospice principles preferred
