What Youll Do
Manage a caseload of hospice patients in homes assisted living communities and skilled nursing facilities
Coordinate care with physicians and the interdisciplinary team
Deliver expert symptom management and clinical oversight
Educate patients and families throughout the hospice journey
Ensure accurate and timely documentation
Qualifications
Active Georgia RN license
Valid drivers license and reliable transportation
Minimum one year of nursing experience
Hospice or home health experience preferred
