You will work with talented professionals who have a passion for our community and quality patient care! Our Hospice clinicians work with physicians to provide in-home rehabilitation to our patients. Together we will help our patients achieve the outlined goals and milestones on their patient care plan.
Job Summary:
We are seeking a compassionate and skilled Hospice - CNA. The ideal candidate will have experience working in hospice care and be able to provide exceptional care to our patients.
Responsibilities:
- Assist patients with daily living activities such as bathing dressing and grooming
- Administer medications as prescribed by the physician
- Monitor patients vital signs and report any changes to the physician
- Provide emotional support to patients and their families
- Assist with end-of-life care
- Maintain accurate and up-to-date patient records
- Collaborate with other healthcare professionals to provide comprehensive care
Requirements:
- Certified Nursing Assistant (CNA) certification
- Experience working in hospice care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and patient-centered approach to care
- Ability to work flexible hours including weekends and holidays
