Job Summary
RN Hospice Nurse. Mon-Thurs on call. Mileage reimbursement. Company equipment. Sign on bonus eligible.
- Serves as a member of the hospice team completing admissions coordinating care and conducting follow-up visits.
- Reviews patient eligibility and collaborates with the interdisciplinary team Medical Director and attending physician to develop care plans.
- Provides compassionate patient-centered care to individuals with terminal illness across care settings.
- Performs comprehensive assessments and develops individualized plans of care.
- Administers treatments and medications accurately and documents care in electronic medical records.
- Advocates for patients and supports their rights and psychosocial cultural and emotional needs.
- Participates in interdisciplinary team meetings and collaborates across care teams.
- Contributes to performance improvement initiatives and supports positive patient outcomes.
- Utlizes clinical expertise in dynamic and changing patient conditions.
- Implements strategies to improve care efficiency outcomes and resource utilization.
- Serves as a role model and provides support to staff students and team members.
- Uses problem-solving skills to address patient care and operational challenges.
- Provides feedback to staff and participates in shared decision-making activities.
- Maintains strong critical thinking and organizational skills to manage competing priorities.
- Uses electronic systems to support communication documentation and continuity of care.
- Collaborates with billing to ensure authorization requirements and patient financial responsibilities are addressed.
- Demonstrates flexibility in scheduling and prioritization of patient care needs.
- Ensures compliance with hospice policies procedures and regulatory requirements.
- Maintains accurate and timely documentation in the electronic medical record.
- Participates in hospice-related events and community support activities.
Education: Graduate of an accredited register nurse program.
Experience: 1 year experience in hospice home health oncology or medical surgical nursing required.
Licensure Certification and/or Registration: Currently licensed as a Registered Nurse in the state of Virginia or approved compact state. Must have valid drivers license automobile insurance and personal vehicle for making home visits. Life Suppor t: Must obtain/maintain AHA BLS-HCP.
Other Minimum Qualifications: Keyboarding skills required for automated computer systems. including electronic medical records.

Compensation Range:
$76689.60 - $115044.80Employee pay is based on a variety of factors including qualifications credentials education and prior work experience. The compensation range does not include benefits or pension plan.
