What Youll Do

As the Hospice Nurse Liaison youll play a key role in connecting patients families hospital providers and the Granite VNA hospice team by:

• Building and maintaining strong relationships with physicians nurses case managers social workers and other key contacts at a local hospital.
• Serving as the on-site ambassador for Granite VNAs hospice program and promoting awareness of our services.
• Guiding referrals by facilitating the hospice intake process and transferring essential referral information from the hospitals electronic medical record (EMR) to our Intake team.
• Maintaining a visible presence throughout the hospital and serving as the go-to resource for hospice referrals and questions.
• Assessing patients and reviewing medical records to determine hospice appropriateness while conducting hospice informational visits and General Inpatient (GIP) hospice admissions as needed.
• Educating patients and families about hospice philosophy eligibility and goals of care with empathy and compassion.
• Partnering closely with the Palliative Care Team to identify patients who may benefit from hospice services.
• Ensuring seamless communication between hospital staff hospice clinicians facilities and community partners.
• Collaborating with the General Inpatient (GIP) Nurse to provide responsive service coordinate smooth transitions and ensure continuity of care.
• Completing accurate and timely documentation.
• Monitoring hospital partner satisfaction and working with the Director of Community Engagement to strengthen relationships and improve service.
• Using critical thinking to navigate complex cases educate referral sources and identify opportunities for process and quality improvement.
• Coordinating with vendors to arrange durable medical equipment and infusion therapies when needed.

This is a full-time position with a schedule generally occurring: MondayFriday 8:00 a.m. 4:30 p.m.

• Graduate of an accredited school of nursing accompanied by current New Hampshire nursing license or compact state license through the Nurse Licensure Compact (NLC)
• At least 6 months of hospice related experience preferred
• Previous experience in business development or care coordination
