**Dates:** - Monday Oct 12 (8 AM) Monday Oct 19 (8 AM) - Monday Nov 9 (8 AM) Monday Nov 16 (8 AM) - Potential week in December

**Telemedicine:**- Not applicable for this coverage period.

**Facility Highlights:**-

**Designations:** Level III Trauma Center; accredited Stroke and Spine Center.

- **Cardiac Services:** Comprehensive cardiology program encompassing open-heart surgery diagnostic and interventional procedures electrophysiology TAVR and cardiac rehabilitation. Dedicated ICU/CCU with 30 beds including 10 beds reserved for critical care.

- Infectious Diseases

- Family Birthing Center

- General Surgery

- Hospice and PACE programs

**Qualifications:**

- Active Indiana State License or IMLC eligibility.

- Board Certification: Must be Board Certified or Board Eligible (certification must be acquired within 5 years post-residency).

- Experience: No minimum years required.

- **Vaccinations:** - Flu vaccination: Mandatory unless a medical or religious exemption is provided.

- COVID-19 vaccination: Mandatory unless a medical or religious exemption is provided.

- Required Certifications: BLS certification essential for all specialties.

Candidates will be expected to enter their working hours into the Vemsta platform and perform order entry during their locum tenure.
