Our Hospice Social Workers provide compassionate psychosocial support to patients families referring source and team members as they navigate the emotional social and practical challenges of serious illness and end-of-life care. You will serve as an advocate resource and trusted member of the interdisciplinary hospice team helping patients and families receive the support they need throughout their hospice journey.

• Social Work degree that complies with CHAP and state of residence regulations.
• Assess patients and families psychosocial emotional spiritual and socioeconomic needs and incorporate findings into the individualized plan of care.
• Counsel patients and families while providing emotional support crisis intervention and guidance as they navigate illness loss grief and significant life transitions.
• Educate patients and families about available community resources and connect them with appropriate programs services and support.
• Advocate for patients and families to identify barriers to care and the hospice team to develop appropriate solutions to ensure their needs preferences values and goals are heard and addressed.
• Participate in interdisciplinary team (IDT) meetings and collaborate with physicians nurses chaplains aides facility staff and other members of the care team.
• Maintain accurate thorough and timely documentation in accordance with hospice standards and regulatory requirements.
