Hospice Liaison LVN

A licensed vocational nurse is needed to serve as a Hospice Liaison combining clinical expertise with outreach and relationship-building. The role focuses on expanding awareness of hospice services supporting referrals and improving access for patients and families.

• Monday to Friday 8:00 AM 4:30 PM
• Territory: Marin County California

• Build and maintain relationships with referral sources
• Identify new referral opportunities to support program growth
• Strengthen ongoing partnerships within the community

• Conduct initial discussions with patients and families
• Explain hospice services and care philosophy clearly and compassionately
• Support consent and admission processes

• Collaborate with liaison teams to gather patient clinical information
• Assist with admissions and community outreach activities
• Serve as a clinical resource for sales and referral partners

• Participate in outreach and awareness activities
• Support efforts to position hospice services as a preferred choice in the service area

• Maintain accurate records and CRM updates
• Submit timely reports to support regulatory and operational requirements

• Identify key accounts and provide feedback on opportunities and challenges
• Share insights with leadership to support growth strategy

• Active unrestricted LVN/LPN license
• At least 1 year of hospice clinical experience (23 years preferred)
• Prior experience in sales or customer service preferred
• Valid drivers license with insured reliable vehicle

• Hourly range: $40 $45 (based on experience and responsibilities)
