Trustbridge Hospice a proud member of Empath Health is seeking a compassionate and dedicated Registered Nurse (RN) to join our team at our Boca Raton Inpatient Care Center. You will work the following schedule: 7pm - 7:30am and every other weekend.
Since 1978 Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate community-based care. Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County we continue to provide comfort dignity and peace to those facing advanced illness.
What youll Do:
As an RN in our inpatient care center you will play a vital role in coordinating and delivering compassionate end-of-life care:
- Provide high-quality nursing care in alignment with the organizations mission vision and values
- Coordinate patient and family care in collaboration with the primary physician and interdisciplinary team (IDG)
- Conduct thorough assessments of patient and family needs
- Develop and implement individualized Interdisciplinary Care Plans
- Ensure timely and coordinated delivery of interventions and services outlined in the care plan
- Deliver personalized sincere and engaging care to patients and their families
- Address concerns promptly to ensure comfort trust and satisfaction
- Maintain the highest level of confidentiality and respect in all aspects of care
What Youll Need
- Registered Nurse currently licensed in the State of Florida.
- BSN preferred
- Hospice certification preferred.
- Current Basic Life Support Certification.
- One year in public health home nursing care oncology nursing medical/surgical nursing geriatric nursing or Hospice nursing preferred.
- Understanding of and ability to apply characteristics of Hospice Nursing: case management crisis intervention communication skills teaching skills thanatology skills and needs
- Basic computer skills and EMR software experience a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida drivers license.
