Hospice - Full Time On-Call - Registered Nurse (RN)

Job Summary:
As a Hospice RN you will be responsible for traveling to several homes a day to administer healthcare services and help patients maintain their independence. Duties may include administering at home IVs changing dressings cleaning wounds and foley/catheter insertions.This is a Full-Time On-Call RN position. The work schedule will follow a 7 days on / 7 days off rotation.
Responsibilities:
- Case management and supervised visits.
- Perform skilled and comprehensive assessment of the patient during the initial admission.
- Assess and chart observations of the patients condition at each visit (vital signs.
- Provide skilled nursing care as needed in accordance with the plan of care (administering medication cleaning/dressing wounds assessing the healing progress).
- Provide proper patient education for home health care procedures and strategies.
- Responsible for the overall clinical record maintenance to include visit note (must be completed within 24 hours) comprehensive assessments including OASIS (must be completed within 48 hours) medication profile orders/plan submitted in a timely manner etc.
- Maintain confidentiality of patient PHI through HIPPA law.
- Keep the patient comfortable through palliative care
Qualifications:
- Current Texas State license as a Registered Nurse.
- Minimum 3 years of Hospice experience required
- Current CPR certification.
- Reliable transportation.
- Texas Drivers License and Auto Liability Insurance.
- Current TB Skin Test
- EMR experience (Kinsser preferred).
