Job Summary

SUMMARY
Provide personal care under the supervision of a nurse according to the patients plan of care.
Collaborate with transdisciplinary team in a variety of settings.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide personal care and assist with ADLs according to the Plan of Care - POC.
  • Provide safe environment for patients.
  • Safely ambulate/transfer patient as appropriate.
  • Assist with passive and active ROM exercises as directed by the nurse.
  • Prepare nutritional snacks as appropriate for the patient.
  • Under the direction of the nurse perform simple wound care skin care and bowel care
    protocols according to the plan of care where appropriate.
  • Recognize and promptly report any changes in the mental or physical status of the
    patient to the nurse.
  • Record in clinical record on appropriate documents the activities provided and
    observations of patient at the time of visit.
  • Complete all charting timesheets and mileage logs as defined by the agency policy and
    procedures.
  • Attend and actively contribute during patient care conference.
  • Participate in HNA discipline meetings team meetings staff meetings retreats and in-services as appropriate.
  • Attend in-services to obtain 3 education hours per quarter.(need 12 a year)
  • Participate in the on-call rotation as scheduled (Home Care only).
  • Assist the Team Leader with other designated activities as necessary (scheduling supply
    organization answering office telephone etc.)
  • Demonstrate positive customer-service responsiveness according to agency standards.
  • Flexible in meeting patient/family needs that require variable shifts and weekends.
  • Participate in chart review quality improvement activities and other meetings as
    assigned.
  • Participate in coordination of and communication with volunteer staff.
  • Provide basic caregiving education to caregiver/family members such as turning and
    positioning transfer techniques and changing linens
  • Complete annual competency requirements.

QUALIFICATIONS

(EDUCATION and/or EXPERIENCE)

  • Must be state tested nursing assistant in good standing and have 12 months of experience
    as a nursing assistant.

CERTIFICATES AND LICENSES

  • Must be a Florida Certified Nursing Aide-CNA.
