As a Clinical Team Manager youll contribute to our success in the following ways:

• Oversee patient care services staff assignments referrals and care coordination to ensure patient needs are met.
• Plan and coordinate Home Health services with patients families physicians providers vendors and facilities.
• Ensure timely start of care and ongoing compliance with applicable regulatory guidelines.
• Review and approve individualized Plans of Care physician orders and proposed changes for clinical appropriateness.
• Conduct regular OASIS reviews to support accurate documentation and quality patient care.
• Provide ongoing guidance coaching and support to direct care team members and assist with interviewing and hiring clinical staff.
• Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations including field work as needed.

What is Required

• Associate degree in a Nursing related field Bachelor preferred
• 2 years of home care experience in Home Health or Hospice environment
• 1 year of supervisory experience required
• Current unrestricted RN license valid for the state of work
• Ability to quickly become proficient in enterprise applications such as Homecare Homebase Workday and have knowledge in Microsoft Office
• Ability to frequently lift push pull and support up to 50 pounds including positioning or transferring patients and moving equipment
• Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
