As a Clinical Success Mentor - Hospice youll contribute to our success in the following ways:

  • Maintains current knowledge of Medicare/Medicaid hospice third-party payor regulations and applicable federal state and local requirements.
  • Provides training and coaching on hospice eligibility criteria regulatory standards and disease-specific guidelines.
  • Educates clinical staff and physicians on medical functional prognostic and documentation requirements supporting hospice eligibility.
  • Trains and supports interdisciplinary teams (IDG/IDT) on roles responsibilities collaboration documentation and regulatory compliance.
  • Conducts ongoing education on regulatory changes quality standards audits and hospice best practices.
  • Assesses clinical quality across nursing social work and spiritual care through IDG observation audits and eligibility scoring.
  • Identifies opportunities for improvement and provides coaching training resources and follow-up to strengthen clinical quality and compliance.
  • Attends IDG meetings across branches to assess processes and provide guidance to improve quality and compliance.
  • Supports key organizational initiatives including live discharges eligibility and documentation and IDG process improvement.

What is Required

  • Bachelors degree or Graduate of an accredited program of Nursing.
  • Current State License as a Registered Nurse with a Compact Licensure preferred.
  • Minimum of three to five (5) years relevant experience as a registered nurse in a Hospice environment with quality compliance and/or leadership experience preferred.
  • Must possess a valid drivers license maintain an acceptable driving record OR have reliable transportation to travel between patient homes and assigned work locations.
  • TRAVEL: 50-75% travel.
  • Meets all applicable agency policies and procedures related to health screening and required testing.
