Hospice Clinical Liaison (RN) WATCH US GROW!

Summary:

We are seeking an experienced and relationship-driven Hospice Clinical Liaison to serve as a trusted clinical resource to hospitals and other healthcare partners.

Qualifications:

• Graduate of an accredited school of nursing with a current Texas license as a Registered Nurse.
• Current certification in basic life support.
• Excellent organizational skills to handle a variety of tasks simultaneously.
• Effective interpersonal skills.
• Excellent communication skills both oral and written.
• Ability to accommodate multi-faith beliefs and backgrounds of patients/families.
• Ability to function as part of an inter-disciplinary team.
• Ability to travel to multi-work locations during workday.
• Must have reliable insured vehicle and valid Texas Drivers license.

Duties/Responsibilities:

• Develop and maintain strong relationships with hospitals physicians case managers social workers skilled nursing facilities assisted living communities and other healthcare partners
• Serve as a clinical resource and subject matter expert regarding hospice philosophy services eligibility criteria and the referral and admission process
• Identify patients who may benefit from hospice services and collaborate with referral sources to facilitate timely evaluation and referral
• Review available clinical information to help determine potential hospice appropriateness and coordinate additional clinical review when needed.
• Educate patients families caregivers and healthcare professionals about hospice care available services and what to expect throughout the admission process.
• Respond promptly to referrals and inquiries and maintain consistent communication with referral sources throughout the referral process.
• Partner closely with Admissions Clinical Operations Business Development and other internal teams to remove barriers and support timely access to care.
• Conduct educational presentations in-services and one-on-one education for healthcare professionals and community partners.
• Maintain an active presence within assigned hospitals facilities or accounts and develop trusted relationships with key clinical and operational stakeholders.
• Identify opportunities to strengthen referral relationships and increase awareness and understanding of hospice services.
• Ensure referral activities documentation and communications are completed accurately and in accordance with organizational policies and applicable regulatory requirements.
• Represent the organization professionally and compassionately within the healthcare community.
