• $90000 $135000 Depending On Experience

Responsibilities:

• Average Case Load Of About 8 Patients
• Monday Friday 8 am - 5 pm And On-Call
• Lead And Supervise The Clinical Team To Ensure High-Quality Compassionate Care
• Oversee Patient Care Delivery Clinical Documentation And Regulatory Compliance
• Conduct And Participate In IDT Meetings To Coordinate Care Plans
• Serve As A Clinical Resource For Staff Patients And Families
• Train Mentor And Evaluate Nursing And Support Staff
• Ensure Accurate Timely Documentation And Adherence To Medicare And State Guidelines
• Collaborate With Physicians And Healthcare Partners To Ensure Continuity Of Care
• Support Quality Improvement Initiatives And Assist With Audits And Surveys
• Manage Clinical Schedules And Ensure Adequate Staffing Coverage
• Address Concerns From Patients Families Or Staff With Professionalism

Requirements:

• Active RN License In The State Of Oklahoma
• 2 Years Of Clinical Experience (Hospice Or Home Health Preferred)
• Prior Leadership Or Supervisory Experience
• Strong Knowledge Of Medicare Regulations And Clinical Best Practices
• Excellent Communication Assessment And Organizational Skills
• Proficient In EMR Systems And Documentation Standards
• Current CPR Certification
