Shift Options:Days (10:00 AM - 7:00 PM OR 11:00 AM - 8:00 PM). Schedules offer flexibility based on patient availability.

• Schedule A:Sunday- Thursday
• Schedule B:Tuesday- Saturday
• Weekends:One weekendper month required.

Job Summary

The Hospice Admission Visit RN is responsible for admitting patients to hospice service managing critical intake workflows and carefully assessing the physical psychosocial environmental safety and developmental needs of patients and their families. This field-based clinical role coordinates care plans with a multidisciplinary rehabilitation team and conducts necessary follow-up visits to support high-quality care.

Key Responsibilities

• Admit patients into servicesby coordinating clinical intake procedures and providing immediate crisis-intervention assessment.
• Conduct comprehensive physical and psychosocial evaluationsof end-of-life patients carefully monitoring safety and environmental needs.
• Develop and modify individualized care plansin partnership with physicians social workers and the interdisciplinary hospice team.
• Maintain precise clinical electronic chartingand tracking data within the Homecare Homebase (HCHB) system.
• Participate in the hospice organizationscontinuous Quality Assessment Performance Improvement (QAPI) tracking programs.
• Teach and guide family caregiverson symptom management medication administration and emotional support tools.

Territory Coverage Coverage

Candidates must be prepared to conduct field visits across three regional counties:

• San Francisco County
• San Mateo County
• Santa Clara County

Mandatory Requirements

• Experience:Minimum of 1 solid year of active recent Hospice clinical experience is strictly required.
• Licensure:Active and unrestricted Registered Nurse (RN) license in the state of California.
• Life Support:Active AHA Basic Life Support (BLS / BCLS) professional certification.
