As a Home Health Speech-Language Pathologist youll help patients overcome challenges related to speech language cognition communication and swallowing while improving their quality of life in the place they feel most comfortabletheir home.

Every visit is an opportunity to help someone communicate more clearly eat more safely think more confidently and regain control over their daily life.

• Complete speech language cognitive and swallowing evaluations
• Develop individualized treatment plans based on patient needs and goals
• Provide skilled speech therapy services in patients homes
• Educate patients and caregivers on communication and swallowing strategies
• Collaborate with physicians and interdisciplinary team members
• Monitor patient progress and adjust treatment plans as needed
• Maintain timely and accurate clinical documentation

• Active Virginia Speech-Language Pathologist License Required
• Certificate of Clinical Competence (CCC-SLP) Preferred
• Valid Drivers License and Reliable Transportation
• Strong Communication and Documentation Skills
• Home Health Experience Preferred but Not Required
• Passion for Delivering Exceptional Patient-Centered Care
