Speech Language Pathologist SLP - Home Health
Per Diem - Flexible Schedule
BAYADA Home Health Care is currently seeking an experiencedSpeech Language PathologistSLP for a per diem opportunity to perform home health visits for adult and geriatric patients in their homes in Richmond North Kingstown Westerlyand Surrounding Towns. Prior home care experiencepreferred but not required.
As a homehealthSpeechTherapist you will bean integralmember of a multi-disciplinary health care teamthat providesrehabilitative care and skilled nursing to clients affording them the opportunity to receive the medical care required to remain at home.

Responsibilities include:
- Make home visits to clients in designated geographic territories.
- Perform thorough evaluations to assess diagnose and provide therapeutic interventions for clients who are experiencing speech language communication or swallowing difficulties.
- Develop a treatment plan based on the evaluation physicians orders and recommended goals.
- Continually assess and revise the speech therapy care plan and participate with BAYADA Clinicians in the multidisciplinary care plan as appropriate.
- Educate and instruct patients family members or other patient representatives in rehabilitative care and activities necessary to promote the clients health safety and independent living.
- Accurately document observations interventions and evaluations pertaining to patient care management and services provided utilizing a touch pad tablet.
Qualifications include:
- A current license as a Speech-Language Pathologist in Rhode Island
- One year experience as a Speech Language Pathologist
